First create your form in TEMPLATES >> FORMS.  Once you have created it, now it is ready to apply to a lead or job.  In the lead or job:

1. Click the forms tab. 

2. Click the drop down and choose your form.

3.  It will now prompt you with an option to add the form and click ADD.

4. From there you can send, print the PDF, open in a new window, or edit it before sending.  For now, let's just click send, and it will pop up with an email window that gives you the change to edit your email before you send over or apply the right canned email. 

5. And done!
